Health Attestation Tools


Wellness Screening in Qmlativ and SMS 2.0
Skyward’s Health Attestation tool consists of wellness screening questions for students and staff, and is available in Student/Family Access, Employee Access, Administrative Access, and through the mobile app. This tool also has reporting capabilities.

The default screening questions follow the Center for Disease Control’s (CDC) guidance, but you can customize these questions to meet your district’s needs and to match the guidance from your local health agency. The questions can be completed daily, and once the student or staff member answers the questions, they are approved to enter school or advised to stay home.

Microsoft Return to School
Microsoft Return to School enables daily health attestation, periodic testing, manual contact tracing, and visual dashboards to help schools create a safe return to in-classroom learning. Improve your decision making with integrated data that helps you manage the safety of your students and staff.

With Return to School you can create daily passes (for health attestation), manage testing plans and appointments, enable contact tracing, track daily passes, monitor test results and active cases, and oversee manual contact tracing. The Return to School Portal app is designed as a self-service tool for students, parents, guardians, and staff, and the School Management app gives school administrators the tools they need to monitor and manage the safe return of students and staff to school.

Microsoft Return to School is offered at no-cost for K-12 schools and school districts in Washington State, and is integrated with Skyward’s software.
